All appointments will be subject to a $25 cancellation fee if SPI is not notified at least 24 hours in advance of the appointment. The fee will be applied to each appointment scheduled with a separate provider (i.e. Dr. Spears, physical therapist, orthotist, and performance testing technician are separate providers). 

This policy has been instituted in consideration of all of our patients who wish to be cared for in the limited time our staff has each day. One person's missed appointment could be another person's much needed visit.

We encourage you to take a printed confirmation of your future appointments before you leave the office. Thank you for your cooperations and consideration.

Late Arrival:

In order to serve our patients in a timely manner, if you arrive more than 15 minutes after your scheduled appointment time, you must reschedule that appointment for another time. If you anticipate such a lateness and call ahead of time, we may be able to accommodate you; however, THIS IS NOT A GUARANTEE. You may still need to reschedule that apointment. The cancellation fee applies to all such appointments we are forced to reschedule as outlined above.

This policy is necessary so that our providers can give you the proper attention and the best care possible.

Bringing Children:

Children under the age of 12 are not allowed to accompany parents to their PT appointments. We also do not allow children under the age of 12 in the waiting area, unless accompanied by another adult, while parent is in PT for safety and liability reasons.


You should call your insurance company and inquire if Teddie D. Spears is a contracted provider. You may also inquire when you call the clinic. We may not have your particular plan on our list, in which case you have a lesser know plan and should call your benefits representative to confirm coverage. A confirmation of provider/insurance contract is not a guarantee of coverage.

There are many plans whose coverage differs from one employer to the next within a single insurance company. We strongly suggest that you call your benefits representative prior to making an appointment. Similarly, certain plans may cover some services (i.e. orthopedic specialist consultation) at one rate but other services (i.e. physical therapy) at another rate or not at all. We do not wish you to incur expenxes of which you were unaware. However, you, as the patient, are ultimately responsible for any costs not covered by insurance. Please help us to help you avoid unnecessary expenses.
